Who may enter: Open to all residents residing in the states of: AZ, CO, IL, IN, KS, MN, MO, OH, TN and WI, age 18 and older who are not current Sentry Insurance customers. Employees of Sentry Insurance a Mutual Company (“Sweepstakes Party”), their affiliate companies, subsidiaries, advertising and promotion agencies, judges and their family or household members, are not eligible. 
How to enter: To enter for a chance to win, follow the instructions contained in the Sweepstakes advertisement you received. Or, fill out a piece of 8 ½ x 11 paper with your name, address (including Zip code), home and work telephone numbers (including area codes), e-mail address, date of birth and fax it to: 1-800-632-9947 Attn: Sentry Insurance August 2009 Midwest Airlines Sweepstakes 10-45-14-1. All entries whether received by Internet, phone or fax must be received by 4:59 p.m. CST on December 31, 2009. Internet entries will be deemed made by the authorized account holder of the e-mail address at the time the entry is submitted. The authorized account holder is the natural person who is assigned to the e-mail address by an Internet Service Provider or other organization that is responsible for assigning e-mail addresses or the domain names associated with e-mail addresses. Sweepstakes Party is not responsible for lost, late, misdirected, stolen, damaged, incomplete or illegible entries. Limit one entry per household. 
How to win: On or about January 8, 2010, a random drawing will be conducted by a Sentry Insurance representative to select the winner from all eligible entries received. On or about January 8, 2010, the winner will be notified by telephone. The winner must meet the eligibility requirements set forth in these rules, and sign an affidavit of eligibility, and liability and publicly release, where permitted by law, in order to qualify for the prize. Decisions of Sentry Insurance are final in all matters relating to this Sweepstakes. 
Odds of winning: The odds of winning depend upon the number of eligible entries received. 
Prizes: One (1) Grand Prize of a Nintendo Wii™ package and 100,000 Midwest Miles. All service charges or other expenses not specified herein are the responsibility of the winner. The approximate retail value of this prize is: $2500.00. The prize is non-transferable, with no cash redemption or equivalent. Sweepstakes Party reserves the right to substitute a prize or prize component of equal or greater value should the prize or any component of the prize become unavailable. All taxes on the prize are the sole responsibility of the winner. By participating in this Sweepstakes, entrants agree to release and hold harmless the Sweepstakes Party, its advertising and promotions agencies and each of its respective parent companies, affiliates, subsidiaries, officers, directors, agents and employees, from any and all liability whatsoever for any injuries, losses or damages of any kind arising from or in connection with the 1.) awarding, acceptance, receipt, possession, use and/or misuse of the prize awarded herein; or 2.) participation in the Sweepstakes or any prize related activities, including but not limited to traveling to or from any prize-related activity. 
Claiming prize: The winner must complete, sign and return an affidavit of eligibility and liability/publicity release, where permitted by law, within 10 days of receipt of written prize confirmation or prize will be forfeited and an alternate winner selected. Any unclaimed prize will not be awarded. By accepting prize, winner consents to use of their name and/or likeness for all legitimate business purposes including advertising and promotional activities without additional compensation, unless prohibited by law.

All entries become the property of Sweepstakes Party and will not be returned. By participating, you agree to abide by and be bound by these Official Rules and the decisions of Sentry Insurance, which shall be final with respect to all issues relating to this Sweepstakes. It is your responsibility to ensure that you have complied with all of the conditions contained in the Official Rules.
