Report a Claim
If you would like to report a claim for an individual life policy or annuity, please call 1-800-272-0533 to begin the claims process.
Please have the following information available when you call:
- Name of the deceased
- Date of death
- Cause of death
- Name and address where you wish the claim forms sent
- Name and address of employer if insurance is provided by employer
The following information if provided upon the initial call will help the claims representative answer any questions you may have:
- Deceased's Social Security Number
- Policy number
- Name of beneficiary(ies)
- Address of beneficiary(ies)
